A cactus fell on my hand!! What should I do????? HELP!?

I was moving a cactus in a pot and the pot broke and fell on my hand. My thumb is covered with little thorns. The majority i have gotten out with tweasers, but some are still stuck under my skin. My thumb has swelled up and i can barely move it, it hurts to much. What should I do? Let it go, or go to a doctor, or what??? THANKS!!
